Upon detection/confirmation/identification of an anomaly, personnel shall initiate the following sequence:

This written application is for the prompt declassification of all personnel logs pertaining to Site Ninety-Six. These files are believed to contain crucial information regarding former activities. The release of this information is important for research the scope of Site Ninety-Six's work. A comprehensive justification for declassification will be provided upon demand.
Incident Report: Site 96 Containment Breach
On [DATE], at approximately [TIME] local time, a containment breach occurred at Site 96. Initial reports indicate that Entity/Specimen escaped/broke free/was released from its designated cell/compartment/holding area.
